I give my child permission to attend field trips such as, but not inclusive to, away games and other band functions that require the use of school transportation. I further recognize that my child is representing North Gwinnett High School at all times when away from school for band, and he/she is instructed to comply with regulations of the school, teachers, sponsors, and chaperones who are in charge of the band. I agree to assume responsibility for any unforeseen accident that might occur to my son/daughter during travel or participation in the band. I authorize any emergency medical treatment or first-aid, administered by a certified person, if it becomes necessary. *

I also understand I am responsible for the entirety of the Marching Band fees according to the Payment Schedule provided/on the "Pay Dues" tab. All checks should be made out to NGBPA. Payments can also be made directly on our website. Middle school students can give their checks to Mr. Downs or Dr. Johnson. High School Students can give their checks to Mr. Soloff or Mr. Gibson. *
